Two things are happening at once here and they are on separate controls. The pad is ribbed and covered in raised nubs, sitting right where you grind against it. The bulbous shaft rises out of it and rotates. How to use it
Press the suction base onto a clean, smooth, flat surface and check it has actually gripped before you put weight on it. A tile floor, a shower wall, the side of a tub or a hard chair seat all work. Textured or porous surfaces do not. Then straddle it and set your own pace.
Care
Wash with warm water and a mild soap or a toy cleaner before and after use. It is fully submersible, so washing it under the tap is fine, but dry it completely before storing. Water-based lubricant only, because silicone lubricant degrades silicone.

Yes, and they are on separate controls, so you can run one without the other or both at once.
